Step 1

Step 2
Cast Maddening Cacophony (paying kicker if you choose), putting it on the stack and letting it resolve; when it resolves it moves the specified number of cards from each opponent’s library into that opponent’s graveyard.
Step 3
As Maddening Cacophony moves multiple cards into an opponent’s graveyard, each individual card put into that graveyard counts for the delayed triggered ability; the delayed ability will trigger once for each card put into that opponent’s graveyard this turn per resolved activation of the Guildmage’s {1}{U}{B} ability (see Ruling [2013-01-24] and rule ).
Step 4
All resulting triggers are placed on the stack (multiple triggers if multiple activations created multiple delayed abilities, and multiple triggers per card if multiple delayed abilities exist) and will cause the appropriate life loss when they resolve.
Resolution
When Duskmantle Guildmage’s {1}{U}{B} ability resolves it creates a delayed triggered ability that lasts until end of turn; each time a card is put into an opponent’s graveyard that turn that delayed ability will trigger once per card (Ruling [2013-01-24] and rule ). Therefore, if you activated the Guildmage’s ability once and then cast Maddening Cacophony (not kicked) in a two-player game so the opponent mills eight cards, the delayed ability will trigger eight times and that opponent will lose 8 life. If you had activated the Guildmage’s ability twice earlier (resolving both activations), Maddening Cacophony milling eight cards would cause 16 triggers and 16 life loss (Ruling [2013-01-24] and rule ).
