Overview
When Maddening Cacophony resolves it immediately puts cards from each opponent’s library into their graveyard; Fraying Sanity’s triggered ability (an Aura attached to a player) counts all cards put into that enchanted player’s graveyard during the turn and will cause that player to mill that many cards at the beginning of each end step. You must declare (and pay) Maddening Cacophony’s kicker as you cast it . Fraying Sanity’s trigger will count the cards put into the graveyard by Maddening Cacophony even if Fraying Sanity was not on the battlefield at the time those cards were put there [Ruling 2017-07-14].
Step 1
Cast Maddening Cacophony as a sorcery; if you want it kicked you must announce and pay the kicker as part of casting it .
Step 2
When Maddening Cacophony resolves, each opponent mills the specified number of cards immediately (8 normally, or half their library rounded up if it was kicked). Those cards are put into their graveyards as part of the spell’s resolution.
Step 3
Fraying Sanity’s triggered ability bases X on the number of cards put into the enchanted player’s graveyard from anywhere that turn; the mill from Maddening Cacophony is included in that count (per Ruling [2017-07-14]).
Step 4
At the beginning of each end step, if Fraying Sanity is on the battlefield attached to that player, its triggered ability will trigger and go on the stack; when it resolves it mills X cards, where X is recalculated as the number of cards that have been put into that player’s graveyard so far that turn .
Step 5
The milling done by Fraying Sanity increases the total number of cards put into that graveyard for the rest of the turn, so future Fraying Sanity end-step triggers that turn (and on later turns) will use the larger total when they resolve. If there are fewer cards in the library than the ability attempts to mill, the remaining cards are put into the graveyard; a player only loses for drawing from an empty library, not for having their library emptied by milling .
Resolution
When Maddening Cacophony resolves it mills the opponent(s) immediately; those milled cards are included in the number X that Fraying Sanity uses to determine how many cards the enchanted player mills at the beginning of each end step. If Cacophony was kicked, its larger mill is likewise included. You must declare any kicker when casting the sorcery . Fraying Sanity’s triggered ability will only trigger at the beginning of an end step if Fraying Sanity is on the battlefield attached to that player at that time ; however, the count X includes cards put into that player’s graveyard earlier that turn even if Fraying Sanity wasn’t present when they were put there [Ruling 2017-07-14].
